Are you on medication for Diabetes?

The Diabetes MedsCheck and Diabetes Clinic are free for people with Type 1 or Type 2 Diabetes. Just bring in your Ontario Health Card.


 

Who will I work with?

You’ll work one-on-one with your pharmacist at The Village Pharmacy. 

All our pharmacists have a certification in diabetes education (CDE designation from the Ontario College of Pharmacists). They will conduct the Diabetes MedsCheck and will be your Diabetes health coach for the 3-month program.

The Diabetes Coaching Program:

The diabetes coaching program is a 3-month program. It includes a health and medication assessment, personalized nutrition and exercise plans, coaching, and regular check-ins. Plus, you’ll get A1C testing at the beginning and end of the program so you can measure your progress.

 
 

The assessment includes:

  • A Diabetes MedsCheck

  • A review of your medical history, prescription and non-prescription medications

  • An overview of techniques to optimize your blood glucose monitoring + setting targets for optimal blood sugar levels

  • A1C test to establish a baseline

  • An opportunity for you to share your questions and concerns, side-effects, and discuss your health goals for the program

The personalized health plan is tailored just for you. It’s backed by research on diet, exercise and lifestyle. Plus you’ll get important information on blood sugar monitoring. It includes:

  • A personalized dietary prescription

  • A personalized exercise prescription

  • Blood glucose monitoring best practices

  • Strategies to manage medication side-effects

  • Tips on how to implement these changes into your lifestyle

Checking in with the Pharmacist will give you a chance to:

  • Share your progress and get guidance on any issues / concerns  

  • Review blood glucose monitoring techniques and targets

  • Ask follow-up questions about your medications and side-effects

  • Review and follow-up on the diet and exercise plans

Our expectation is that you will have put into practice the knowledge and strategies from the program to lower your blood sugar, reduce your risk of complications in the future, and feel great. 

You’ll also get to test your A1C again, to measure your progress.

After completing the program, we’ll still be here for you! You can always get in touch with your Pharmacist anytime for follow-up.


How much does it cost?

If you have Type 1 or Type 2 Diabetes, and you have not had a Diabetes MedsCheck in the past year, the Diabetes Clinic at The Village Pharmacy is free with your Ontario Health Card.

If you don’t have an Ontario Health Card, the fee is $100.
